Within 30 days of discharge, the cost of readmission can be very costly. This can add up to thousands of dollars per year in additional costs for an inpatient facility. To help address the need to reduce these costs, Right Track Medical Group is collaborating with inpatient behavioral health providers to enhance success of discharge plans and reduce patient readmission within the first 30 days. Our readmission reduction program (RRP) is designed to quickly engage patients with outpatient care, reduces the likelihood of additional visits to inpatient facilities and puts patients on a path to mental wellness.
What is a readmission reduction program (RRP)?
A readmission reduction program is a collaborative effort between inpatient and outpatient mental health providers that aims to reduce patient readmission to an inpatient facility within the first 30 days following discharge.
How does the program work?
Following discharge from an inpatient facility, our intake team will schedule an outpatient visit within 5-7 days. During the initial visit, our team of psychiatrists, nurse practitioners and therapists will develop an individualized treatment plan based on that patient’s mental health needs. Patients will be engaged using our best-practice model of medication management, therapy and intensive follow-up. Those patients with elevated readmission risk may require a higher frequency of outpatient visits per week.
How can I ensure a patient will participate in the program?
Our team will provide appointment reminder calls to patients for all outpatient visits. We can also provide feedback directly to the inpatient facility to make them aware of any no-shows or withdrawal from treatment prior to achieving outpatient goals.
How can inpatient facilities assist in the success of the program?
Ideally, inpatient facilities will have a staff member dedicated to managing readmission prevention and a solid discharge plan for each patient. Since our efforts are collaborative, effective communication and feedback regarding discharge plans and RRP system improvements from inpatient facilities are welcome and appreciated.
